  1. WHAT INFORMATION Product Evaluations, INC. COLLECTS FROM YOU WHEN YOU VISIT OUR SITE

    Registering with Product Evaluations, Inc., is purely your own choice and is not a requirement. When you register online with ProductEvaluations.com you create a "User ID" which takes the form of your email address and you are then asked to choose a secret password. We then ask for background information such as your name, current email address, street address with zip code, phone number, industry segmentation, menu type, food and beverage sales and average check size. The more information you provide, the better we can customize your on-line experience and choose surveys that fit you specifically. Once you register with productevaluations.com you are no longer anonymous to us.
    Contact and Questions: You may submit questions, comments, or concerns to www.productevaluations.com or at customercare@productevaluations.com. We will ask for your name, email address, your company name, and your comment or question. You may also provide your street address with zip code, phone number, fax number, and Web site.
    Sweepstakes, Contests, and Special Offers: From time to time, we run sweepstakes, contests, and special offers, which you may choose to opt into. Participating in these promotions is purely voluntary. In these offers, we may ask participants for contact information (like your name, street address, email address, industry segmentation), demographic information (like zip code, age, title, or purchase level authority), and perhaps additional information in order to become eligible for special offers.

If you cancel your account, Product Evaluation, Inc. will keep your Individual Member Information accessible to customer service for approximately six months after cancellation.     Information From Contact and Questions: The information you provide with your questions and comments will aid Product Evaluations, Inc. in forming research assumptions, usage patterns, product development and marketing opportunities as a whole but never individually. We will use the information to respond to your inquiry and may share it internally to improve our service to you.
