      One of the basic principles of Product Evaluations, Inc. is that we ask only for the information we need to provide an accurate accounting of who you are, to properly match you
      with the required survey demographics. In our site, we ask you to provide information about yourself by filling out and submitting an on-line form. The features of the Web site
      where personal information is collected are explained below:
Registering for Product Evaluations, Inc., FoodForum Membership:
Registering with Product Evaluations, Inc., is purely your own choice and is not a requirement. When you register online with you create a “User ID”
which takes the form of your email address and you are then asked to choose a secret password. We then ask for background information such as your name, current email address, street
address with zip code, phone number, industry segmentation, menu type, food and beverage sales and average check size. The more information you provide, the better we can customize
your on-line experience and choose surveys that fit you specifically. Once you register with you are no longer anonymous to us.
Contact and Questions: You may submit questions, comments, or concerns to or at We will ask for your name, email address, your company name, and your
comment or question. You may also provide your street address with zip code, phone number, fax number, and Web site.
Sweepstakes, Contests, and Special Offers: From time to time, we run sweepstakes, contests, and special offers, which you may choose to opt into. Participating
in these promotions is purely voluntary. In these offers, we may ask participants for contact information (like your name, street address, email address, industry
segmentation), demographic information (like zip code, age, title, or purchase level authority), and perhaps additional information in order to become eligible for special offers.
Use of IP Addresses: An Internet Protocol (“IP”) address is a number that is automatically assigned to your computer whenever you are surfing the Web. Web servers automatically
identify your computer by its IP address. When visitors request pages from Product Evaluations, Inc., our servers log the visitors’ IP addresses. We do not normally link IP addresses to
anything personally identifiable, which means that a visitor’s session will be logged, but the visitor remains anonymous to us. Product Evaluations, Inc. collects IP addresses for the

Individual Member Information: “Individual Member Information” is the information collected when you register in the My Profile area. Individual Member Information will be

parties. If you cancel your account, Product Evaluation, Inc. will keep your Individual Member Information accessible to customer service for approximately six months after cancellation.
Thereafter, it is placed in Product Evaluations, Inc.’s service bureau archives.
Information From Contact and Questions: The information you provide with your questions and comments will aid Product Evaluations, Inc. in forming research assumptions, usage patterns,
product development and marketing opportunities as a whole but never individually. We will use the information to respond to your inquiry and may share it internally to improve our service
to you. In addition, we may use the information for various business purposes, including, but not limited to, sharing consumer testimonials about our products or services. These testimonials
may be posted on the Product Evaluations, Inc. Web site; however, we will not post them without your prior permission. In addition, we may share your testimonials with third parties; however,
in such a case, we will not share your personally identifiable information, but we will just use your comment or question with your geographic location or other non-personal information.
Information From Surveys: Results from the surveys may be posted on the Product Evaluation, Inc. Web site; however, personally identifiable information will not be posted. Product Evaluations,
Inc. shares the information you provided during registration, surveys, contests, sweepstakes, and special offers, in aggregate form for our clients. Product Evaluations, Inc. also may share
aggregate demographic information with clients of Product Evaluations, Inc. This means we will not disclose individual names, addresses, telephone numbers, email addresses, or other personally
identifiable information unless we have your permission. Currently, Product Evaluation’s surveys are limited to US residents only.
